Job summary
Community Care, Inc. Home Health is seeking a compassionate LPN to provide 1:1 private duty pediatric care for a 12-year-old male with a Trach/Vent in a home setting in Washington, PA. This full-time overnight role features one-on-one patient care and a supportive team.
Responsibilities include ADL assistance, g-tube care, vitals, seizure monitoring, and safe medication administration, while collaborating closely with family and care staff to deliver superior pediatric care.
Qualifications
- LPN license in PA required.
- Graduate of an accredited nursing program.
- At least 6 months of nursing experience; home health preferred.
- CPR certification required.

- Deliver exceptional, family‑centered pediatric care
- Current LPN (or RN) license in the state of Pennsylvania
- Graduate of an accredited nursing program
- At least 6 months of nursing experience (home health preferred, but not required)
- CPR certification
- Reliable transportation and a valid driver's license
- A caring heart, strong independence, and a passion for pediatric care
