Job summary

Community Care, Inc. Home Health seeks a compassionate LPN for 1:1 private duty pediatric care in Trafford, PA. This full-time day shift role includes 3 hours/day on M/W/F and 5 hours/day Tue/Thu, with nights and weekends available.

Join a caring team delivering personalized pediatric care to a 12-year-old patient in a home setting. You will assist with ADLs, tube care and feedings, monitor vitals, administer oxygen and medications, and communicate with the family and care team to ensure safe,

Qualifications

• Current LPN (or RN) license in Pennsylvania.
• Graduate of an accredited nursing program.
• CPR certification.
• At least 6 months of nursing experience (home health preferred).

Responsibilities

• Assist with activities of daily living.
• Perform g-tube care, feedings, and aspiration precautions.
• Monitor vitals.
• Administer oxygen and medications safely.
• Communicate clearly with the care team and family.

What You'll Do

• Reliable transportation and a valid driver's license
