Job summary

AdventHealth seeks a dedicated LPN to support discharge planning, coordinating care from inpatient to post-acute services. You will review medical records, document needs, and advocate for patient rights and options during hospitalization.

You will collaborate with RNs and care managers to ensure smooth discharge, communicating with families and community resources while maintaining accurate documentation and safe practices.

Qualifications

• Strong communication and collaboration with health care team.
• Accurate documentation and record-keeping.
• Hospital discharge planning experience preferred.
• Home care or skilled nursing facility experience preferred.
• Proficient with computers and Microsoft Office.

Responsibilities

• Coordinate discharge planning with patients, families, and care providers.
• Review patient records and document discharge plans, barriers, and post-acute needs.
• Communicate with payors for post-acute care authorization as needed.
• Collaborate with RN Care Manager to integrate clinical, social, and financial factors.
• Arrange for post-acute services and facilities to ensure continuity of care.
