Qualifications

Responsibilities

• Communicates daily with care staff to coordinate discharge plans.
• Discusses discharge options with patients/families and elicits preferences.
• Coordinates referrals to community providers and home health agencies.
• Verifies benefits and completes Medicare/insurance documentation.
• Presents final discharge plan and obtains signatures.
• Completes documentation for discharge services and transfers to facilities.
• Coordinates transportation needs for discharge activities.
• Supports in on-call and weekend coverage.
