Qualifications

• MSW or equivalent from an accredited school of social work.
• Current NJ SW license if working at St. Luke’s Warren Campus.
• May hire per diem BSWs currently in school within 6 months of completing MSW.
• If OB coverage area, membership in the National Organization of Perinatal Social Workers is required.
• Graduate of professional nursing program; RN with PA license; NJ RN license for Warren Campus; Advanced degree preferred; BSN within 3 years; MSN and/or Case Management Certification preferred.

Responsibilities

• Develops a discharge plan addressing psycho-social needs and next steps in the continuum of care.
• Collaborates with patient and family and multidisciplinary team to design discharge plans.
• Manages length of stay by identifying barriers to care and ensuring safe discharge.
• Coordinates utilization of patient and community resources for safe transitions.
• Documents care manager functions using electronic systems.
