Qualifications

• 2+ years discharge planning or care coordination experience in a healthcare setting.
• Case management experience preferred.
• Knowledge of community resources available to older adults.
• Knowledge of medical terminology, charting, and familiarity with a skilled nursing facility.
• Strong written and verbal communication skills.
• Basic knowledge of Microsoft Office and operating computer hardware/software.
• Ability to build rapport with patients and families.

Responsibilities

• Coordinate the discharge planning process with residents and healthcare team.
• Assess discharge needs, including medications, equipment, and post-discharge care.
• Develop individualized discharge plans with residents and families.
• Educate residents and families on care, medications, follow-up, and information.
• Coordinate transportation and arrange post-discharge services as needed.
• Complete discharge paperwork, summaries, medication lists, and instructions.
• Monitor residents' condition during discharge and ensure regulatory compliance.
• Participate in team meetings to discuss plans and barriers.
