Qualifications

Responsibilities

• Request care from referring sources promptly.
• Review patient records for discharge planning per criteria.
• Document discharge plans, barriers, and progression needs.
• Provide patient advocacy and support patient rights during hospitalization.
• Communicate with payors for authorization for post-acute care.
• Coordinate post-acute services and social needs planning.
