Responsibilities

• Provide direct patient care under supervision of RN/physician.
• Administer medications and treatments as prescribed.
• Monitor and record vital signs and report changes.
• Assist with patient assessments and care plans.
• Perform wound care and basic nursing procedures.
• Document patient care in EMR.
• Educate patients and families on health maintenance.
• Assist with admissions, discharges and transfers.
• Maintain a safe, clean environment and follow policies.

Local Pay: $ 49.00/hour

Travel Package:

Travel Rate (based on 36 hours): Hrly $18.04, Stipend (Lodging $875.00 + M&I $413.00) = $1,288.00,

Total Comp :$1,937.44

Travel Eligibility: Candidates must live at least 50+ miles from the facility to qualify for Travel Pay.

Shift Details:

• Days (12 hour shift)

• 36 hours work week

Licensed Practical Nurse (LPN) Duties & Responsibilities:

• Provide direct patient care under the supervision of a Registered Nurse or physician.

• Administer medications and treatments as prescribed.

• Monitor and record patients' vital signs and report changes in condition.

• Assist with patient assessments and implement individualized care plans.

• Perform wound care, specimen collection, and other basic nursing procedures.

• Document patient care accurately in the electronic medical record (EMR).

• Educate patients and families on health maintenance and treatment plans.

• Assist with admissions, discharges, and transfers.

• Maintain a safe, clean, and therapeutic environment for patients.

• Follow facility policies, infection control protocols, and regulatory standards.

Minimum Qualifications

• Current, valid, and unrestricted NY license/certification in good standing, as required for the position.

• Minimum of 1 year of recent experience as a LPN.

• Copy of current, completed Primary Source Verification (PSV) from the appropriate licensing board or verification agency, if required.

• Copy of all current required certifications and compliance documents (Healthcare Provider BLS certification; TB screening with negative PPD, QuantiFERON-TB Gold, or T-SPOT results; Hepatitis B Consent/Declination Form; and proof of Rubella, Rubeola, and Varicella vaccination or titers).
