HAN Staff seeks a Licensed Practical Nurse (LPN) for Franklin, Wisconsin. The position offers local pay and W2 employment, with travel pay opportunities for qualifying assignments.

The role involves direct patient care, meds administration, vitals monitoring, EMR documentation, and patient education in a correctional healthcare setting.

Formación

• Active WI LPN license in good standing.
• 1+ year of LPN experience in last 3 years.
• Primary Source Verification (PSV) may be required.
• BLS and other required certifications (immunizations, TB test, etc.) are current.

Responsabilidades

• Provide direct patient care under RN/physician supervision.
• Administer medications and treatments as prescribed.
• Monitor vital signs and report changes.
• Assist with assessments and care plans.
• Document care in EMR.
• Educate patients and families on health maintenance and treatment plans.
• Assist with admissions, discharges and transfers.
• Maintain a safe, therapeutic environment.
• Follow infection control protocols and facility policies.

Local Pay: $ 43.00 /hour - W2

Travel Pay:

Travel Rate (based on 36 hours): Hourly $18.11, Stipend (Lodging $700.00 + M&I $329.00) = $$1,029.00

Total Comp :$1,680.96

Travel Eligibility: Candidates must live at least 50 miles from the facility to qualify for Travel Pay.

Shift Details:

• 12 hr- Night shift

Licensed Practical Nurse (LPN) Duties & Responsibilities:

• Provide direct patient care under the supervision of a Registered Nurse or physician.
• Administer medications and treatments as prescribed.
• Monitor and record patients' vital signs and report changes in condition.
• Assist with patient assessments and implement individualized care plans.
• Perform wound care, specimen collection, and other basic nursing procedures.
• Document patient care accurately in the electronic medical record (EMR).
• Educate patients and families on health maintenance and treatment plans.
• Assist with admissions, discharges, and transfers.
• Maintain a safe, clean, and therapeutic environment for patients.
• Follow facility policies, infection control protocols, and regulatory standards.

Minimum Qualifications

• Current, valid, and unrestricted WI license/certification in good standing, as required for the position.
• Minimum of 1+ year within the last 3 years of recent experience as a Licensed Practical Nurse
• Copy of current, completed Primary Source Verification (PSV) from the appropriate licensing board or verification agency, if required.
• Copy of all current required certifications and compliance documents (e.g., . BLS, N95 fit test, immunization records, TB test,).
