Job summary

Bethesda Health Group is seeking a licensed Practical Nurse (LPN) to provide basic nursing care and assist with resident assessments at admission and ongoing care. You will document assessments, participate in care planning, and communicate significant changes to the IDT and families.

Qualifications

• Education from an accredited nursing program resulting in becoming an LPN.
• Current LPN license in the state you will be working in.
• CPR and AED certification required or willingness to obtain within six months of employment.
• IV certification required or willingness to obtain within six months of employment.
• Long-term care experience preferred.
• Ability to assess the basic nursing care needs of geriatric residents.
• Excellent communication with residents, visitors and staff.
• Time management and organizational skills required.
• Ability to stand and walk for long periods of time.
• Ability to lift heavy objects with assistance when necessary.
• Ability to transfer and move patients weighing 200+ lbs with assistance.

Responsibilities

• Provide basic nursing care and assist in resident assessments on admission and ongoing basis.
• Communicate changes in condition to the interdisciplinary team (IDT) and residents/families.
• Document assessments accurately and timely; participate in care planning and care plan revisions.
• Administer medications and treatments safely; monitor patient responses.
• Utilize SBAR for timely physician notification and escalate issues as needed.
• Maintain composure during crises and participate in problem resolution via QAPI.
