AltaPointe Health seeks a Care Manager to support behavioral health care coordination, documentation, and care transitions. You will work within an interdisciplinary team to ensure seamless services across internal programs and external providers.

Responsibilities include chart reviews, care coordination, discharge planning, attendance monitoring, and ensuring timely referrals with proper documentation in the Care Manager System.

Qualificações

• Strong knowledge of behavioral health systems (mental health, substance use, social determinants).
• Proficiency in navigating and documenting within electronic health records (EHR).
• Experience with treatment planning, interagency coordination, and client engagement.
• Strong organizational and communication skills with accurate documentation.

Responsabilidades

• Chart Review and Documentation: review records, assess utilization, document in Care Manager System.
• Care Coordination: coordinate physical, behavioral, social health services; facilitate access to community services.
• Hospital Discharge and Transition Support: follow-up within 24 hours; verify follow-up appointments and discharge instructions.
• Service Monitoring and Engagement: monitor attendance; address disengagement; review PHQ-9 and other screenings.
