This in-office, non-face-to-face role supports residents in skilled nursing and long-term care facilities through chart review, screening, care planning, and team collaboration. The position emphasizes documentation, coordination with medical and psychiatric teams, and adherence to HIPAA policies.

Formación

• Active LPN license in good standing.
• Experience with behavioral health screening tools.
• Ability to work in an in-office, non-face-to-face model.
• Strong written and verbal communication.
• Knowledge of HIPAA and patient privacy.
