Job summary

AltaPointe Health Systems seeks a qualified care management professional to coordinate physical, behavioral, and social health services across internal and external providers. You will review charts, monitor engagement, and ensure referrals are completed while maintaining accurate documentation in the Care Manager System.

The role emphasizes risk monitoring, treatment plan updates, and delivering high-quality, compliant care within a multidisciplinary team.

Qualifications

• Bachelor’s degree in behavioral health, human services, nursing, public health, or related field is preferred.
• High School diploma or equivalent with 4 years of experience in behavioral health, care coordination, case management, or related healthcare service delivery is acceptable.

Responsibilities

• Review clinical records to assess service utilization and treatment plan compliance.
• Coordinate physical, behavioral, and social health services across internal programs and external providers.
• Follow up within 24 hours of hospital discharges to ensure continuity of care.
• Monitor client attendance at therapy, psychiatry, and medical appointments.
• Create, follow up, and close referrals in the Care Manager System.
• Monitor client risk levels and support crisis response planning.
• Assist with treatment plan implementation and updates as needs change.
• Manage assigned caseloads and participate in team discussions.
• Ensure documentation meets agency and Medicaid standards.
