Infinite Medical P.C. is seeking a Licensed Practical Nurse (LPN) to serve as a Behavioral Health Care Manager within our Collaborative Care Model.

This in-office, non-face-to-face role supports residents in skilled nursing and long-term care facilities through chart review, screening, care planning, and team collaboration. You will work with a multidisciplinary team including medical providers and psychiatric consultants, focusing on documentation, safety, and quality care within established

Qualifications

• Active and unrestricted LPN license required.
• Graduate of an accredited practical nursing program.
• Strong clinical documentation and medical-record review skills.
• Ability to escalate behavioral health, medication, and patient-safety concerns.
• Proficiency with electronic medical records and tracking systems.

Responsibilities

• Review patient records, diagnoses, medications, behavioral health docs, lab results, and condition changes.
• Document behavioral health screening tools (PHQ-9, GAD-7, MoCA, BIMS).
• Monitor symptoms, treatment response, and progress toward goals.
• Develop individualized behavioral health care plans with the care team.
• Coordinate behavioral health services and follow-up recommendations.
• Communicate findings and care gaps to providers and facility staff.
• Prepare patient case summaries for psychiatric consultant review.
• Maintain CoCM registry and accurate documentation of activities.
