Job summary

Aperion Care is seeking a Licensed Practical Nurse (LPN) to provide direct nursing care to residents and supervise nursing assistants, ensuring high-quality care in accordance with regulations. Responsibilities include planning daily activities, administering medications, and maintaining confidentiality while coordinating with physicians.

The LPN will ensure safety protocols, maintain effective communication with residents, families, and staff, and foster a warm, collaborative environment.

Qualifications

• Must be a Licensed Practical Nurse with a current unencumbered state license.
• Experience in Long Term Care is preferred.
• CPR and First Aid certifications are required.
• Ability to be flexible and adaptable while handling multiple priorities.
• Written and verbal communication skills.
• Organizational and time management skills.
• Demonstrates good judgment, problem-solving, and decision-making skills.

Responsibilities

• Perform all functions under your active license as it pertains to your duties.
• Know the residents you serve and provide care per care plans and physician orders.
• Prepare and administer medications as ordered by the physician.
• Develop and maintain good rapport with inter-departmental personnel to maintain nursing services.
• Create and maintain a warm, calm environment throughout the unit and shift.
• Receive/give nursing report at shift start and end, and accompany rounds with physicians as needed.
• Assist in activities of daily living, meals, and social engagement.
• Communicate with clinical leadership when a resident’s needs change.
• Know how to care for residents with dementia.
• Be willing to take on additional duties within scope of knowledge.
