Monarch Healthcare Management in Minnesota is seeking an LPN Care Coordinator to support the Director of Nursing and RN Nurse Manager in leading a nursing unit. You will assist with care planning, data collection, assessments, and updating care plans, collaborating with an interdisciplinary team to meet resident needs.

The role involves monitoring resident conditions, reporting changes, participating in care conferences, and providing nursing care under RN supervision while helping with staffing

Qualifications

• Must hold a current LPN license in Minnesota and CPR/emergency care certification; background check as per MN DHS.
• Strong communication and functional ability to perform essential job functions with or without accommodations.
• Experience in healthcare settings like skilled nursing or assisted living preferred; familiarity with Medicare coverage.
• Demonstrates multitasking, adaptability, emotional intelligence, active listening and teamwork.

Responsabilités

• Lead and manage a nursing unit by assisting with care planning and resource evaluation.
• Collect resident data and contribute to assessments and care plans (MDS/RAPS).
• Monitor resident conditions, report changes to RNs, participate in care conferences.
• Provide nursing care under RN supervision and support LPNs and nursing assistants; assist with staffing and ensure safe, competent care delivery.
