Job summary

Monarch Healthcare Management in Minnesota seeks an LPN Care Coordinator to support the Director of Nursing, Assistant DON, and RN Nurse Manager in leading a nursing unit. Responsibilities include care planning, data collection, MDS/RAPS assessments, and developing comprehensive care plans in collaboration with the interdisciplinary team.

The role involves monitoring resident conditions, reporting changes to RNs, and participating in care conferences.

Qualifications

• Must hold a current LPN license in Minnesota and CPR/emergency care certification.
• Excellent verbal and written communication; able to perform essential job functions with reasonable accommodation.
• Healthcare setting experience preferred (skilled nursing or assisted living); familiarity with Medicare.
• Strong multitasking, adaptability, emotional intelligence, and active listening.

Responsibilities

• Assist with care planning, resource evaluation, and implementation of the nursing process.
• Collect resident data and participate in assessments such as MDS and RAPS.
• Develop and update comprehensive care plans; collaborate with interdisciplinary team.
• Monitor resident conditions, report changes to RNs, and participate in care conferences.
• Provide nursing care under RN supervision and support LPNs and nursing assistants.
• Assist with staffing needs and ensure safe, competent care delivery; maintain documentation.
• Promote quality improvement initiatives and uphold licensure and ongoing education.
