Job summary

Monarch Healthcare Management is seeking an LPN Care Coordinator to support the Director of Nursing and RN Nurse Manager in leading a nursing unit, coordinating care planning, and ensuring regulatory compliance across a 60+ facility network in Minnesota.

You'll collect resident data, participate in MDS/RAPS assessments, update care plans, supervise LPNs and CNAs, and contribute to quality improvement initiatives while maintaining documentation and constructive teamwork.

Qualifications

• Licensure must be current for Minnesota and CPR certification completed.
• Effective communication in verbal and written form.
• Preferred experience in skilled nursing or assisted living settings.
• Strong multitasking, adaptability, emotional intelligence, and teamwork.

Responsibilities

• Assist with care planning, resource evaluation, and nursing process implementation.
• Collect resident data and participate in MDS and RAPS assessments.
• Develop and update comprehensive care plans; ensure orders are followed.
• Monitor resident conditions, report changes, and participate in care conferences.
• Provide nursing care under RN supervision; support LPNs and CNAs; assist staffing needs.
• Maintain accurate documentation; adhere to policies and regulatory requirements.
• Support quality improvement initiatives and ongoing staff education.
