Monarch Healthcare Management in Minnesota is seeking an LPN Care Coordinator to support nursing leadership in a designated unit. You will assist with care planning, data collection, and coordinating resident assessments under RN supervision.

You will monitor residents, participate in care conferences, help update care plans, ensure documentation accuracy, and support staffing while upholding nursing policies and regulatory requirements.

Qualifications

• Must hold a current LPN license in Minnesota and CPR/emergency care certification.
• Ability to communicate effectively in verbal and written forms.
• Experience in healthcare settings such as skilled nursing or assisted living is preferred.

Responsabilités

• Assist with care planning, data collection, and MDS/RAPS contributions.
• Develop and update comprehensive care plans with the interdisciplinary team.
• Monitor resident conditions, report changes to RNs, participate in care conferences.
• Provide nursing care under RN supervision and support LPNs and nursing assistants.
• Ensure accurate documentation and adherence to policies and regulatory requirements.
• Support staffing needs and promote a positive, inclusive workplace culture.
