Job summary

KalCare Health Care is seeking an experienced LPN to join as Care Coordinator & Clinical Education Coordinator for a 32-hour work week. This role focuses on care coordination, caregiver education, and clinical supervision within our home care program.

You will collaborate with an RN Case Manager, supervise visits, and develop ongoing caregiver training. Ideal candidates are organized, proactive, and skilled at coaching staff and delivering education to improve client outcomes.

Qualifications

• Active Georgia LPN license and CPR certification.
• Strong clinical assessment, observation, and communication skills.
• Experience with electronic documentation and communication systems.
• Proven ability to manage multiple priorities and organize work.

Responsibilities

• Oversee day-to-day clinical coordination of home care.
• Coordinate client Starts of Care and care plans.
• Provide supervisory nursing visits and calendar management.
• Deliver caregiver training and in-services.
• Coach and evaluate caregiver performance and competency.
• Escalate clinical concerns to RN as needed.
• Collaborate with RN Case Manager and scheduling team.
• Develop caregiver education materials and monthly engagement activities.
