Job summary

Hines & Associates, Inc. is seeking an LPN care coordinator to join our nationwide team. The role uses medical knowledge to assess patient needs, develop care plans, and coordinate care with a focus on safety and confidentiality.

This is a full-time position with Monday–Friday hours, initially in-office, with a potential hybrid option after orientation. You will perform telephonic assessments, care planning, monitoring and outcomes measurement within licensure scope.

Qualifications

• LPN with an unrestricted license in the state or a state with licensure reciprocity with the office location.
• Successful completion of case management/disease management orientation.
• Minimum 2 years full-time equivalent direct clinical care to the consumer.
• Excellent communication skills.
• Basic typing with minimum 35 WPM.
• Previous case management experience helpful but not required.

Responsibilities

• Assess patient needs via telephonic assessment to develop an optimum health plan.
• Coordinate and implement care plans, monitor progress and measure outcomes within licensure scope.
• Advocate for patient safety and maintain privacy and confidentiality.
