Patewood Post Acute in Greenville, SC is hiring a Licensed Practical Nurse (LPN) to provide direct patient care in a team-based setting. You will administer meds, monitor residents, and coordinate with CNAs to ensure high-quality care in a skilled nursing facility.

Qualifications

• Active, unencumbered LPN license in South Carolina (required).
• Current CPR certification required.
• Maintain continuing education and stay in good standing with SC Board of Nursing.
• Experience with PointClickCare (PCC) preferred.

Responsibilities

• Deliver direct patient care per care plans.
• Administer medications and treatments as prescribed.
• Monitor resident conditions and report changes to the clinical team.
• Support and supervise CNAs to ensure quality care delivery.
• Maintain accurate, compliant clinical documentation.
