Job summary

Pinnacle Treatment Centers in Kokomo, IN is seeking a Licensed Practical Nurse to join our team. You will work with the Director of Nursing to provide a safe environment and support long‑term recovery for residents, coordinating care and maintaining accurate medical records.

This on‑site role requires Indiana LPN licensure, comfort working independently, and strict adherence to MAT protocols. We offer a competitive salary, full benefits package, 18 days PTO, and opportunities for ongoing

Qualifications

  • Licensed Practical Nurse in Indiana.
  • Must be able to work independently.
  • Must comply with MAT program protocols.

Responsibilities

  • Schedule appointments for tests and doctor visits.
  • Maintain and update medical records and docs.
  • Assist with transport to appointments.
  • Maintain contact with community medical personnel.
  • Manage medical charts for medications.
  • Assist DON with purchasing medical supplies.
  • Monitor detoxification patients and respond to symptoms.
  • Utilize 401k and other benefits.
  • Support ongoing training and certifications.
  • Participate in STAR LRP and tuition assistance.

As an LPN, you play a direct role in saving the lives of our patients each day by serving our residents in a safe environment promoting long term recovery. You will partner with the Director of Nursing and Executive Director to establish and maintain a therapeutic environment and routine for our residents.

Requirements:

  • Licensed Practical Nurse in the state seeking employment
  • Comfortable working independently with the population when needed
  • Compliance with MAT (Medication-Assisted Treatment) program protocols

Responsibilities:

  • Schedule appointments necessary for psychological testing, doctor’s appointments, dental, and eye doctor appointments.
  • Maintain and keep updated all medical records and documentation pertaining to the clients.
  • Assist as needed in transport of clients to necessary appointments.
  • Maintain contact with community medical personnel.
  • Responsible for medical charts for non-prescribed and prescription medication.
  • Assist DON in purchasing of medical supplies.
  • Monitor all detoxification clients and initiate appropriate medical response to detoxification symptoms.
