Job summary

Century Care Management is seeking a Licensed Practical Nurse (LPN/LVN) for full-time day shifts in Jamestown, NC. The role involves direct patient care in a long-term care setting with emphasis on safety and quality of life.

Qualifications

• Graduate of an accredited Practical Nursing program.
• Current, unrestricted LPN/LVN license in the state of practice (North Carolina).
• Knowledge of long-term care regulations, resident rights, and skilled nursing facility standards.
• Experience providing direct patient care to geriatric and long-term care populations.
• Ability to administer medications, treatments, and physician-ordered therapies safely and accurately.
• Skilled in monitoring residents' conditions and reporting changes to the interdisciplinary care team.
• Experience with electronic health records (EHRs) and clinical documentation.
• Knowledge of infection prevention and control practices.
• Strong assessment, communication, and critical-thinking skills.
• Ability to supervise and delegate tasks to CNAs per state regulations.

Responsibilities

• Record a patient’s medical history accurately.
• Take and record measurements of blood pressure, temperature, heart rate, etc.
• Observe patients under treatment to identify progress and side-effects.
• Monitor patients’ condition including fluid intake and output and charting.
• Assist patients in daily activities such as eating and bathing.
• Administer injections, medications, and wound care; assist with basic medical tasks.
• Collect samples for lab testing as assigned.
• Provide emotional and psychological support when needed.
• Communicate with patients’ family to provide advice and release instructions.

• Record a patient’s medical history accurately
