Job summary

Century Care Management in Fayetteville, NC is seeking a licensed practical nurse (LPN) to join our long-term care team. You will record a patient's medical history, monitor vital signs, administer medications, and assist with daily activities to support resident well-being.

The ideal candidate holds an accredited Practical Nursing program diploma and an active LPN/LVN license in North Carolina, with experience in long-term care and proficiency with electronic health records.

Qualifications

• Graduate of an accredited Practical Nursing program.
• Current, unrestricted LPN/LVN license in North Carolina.
• Knowledge of long-term care regulations, resident rights, and skilled nursing facility standards.
• Experience providing direct patient care to geriatric and long-term care populations.
• Ability to administer medications, treatments, and physician-ordered therapies safely and accurately.
• Experience with electronic health records (EHRs) and clinical documentation.
• Strong assessment, communication, and critical-thinking skills.
• Ability to supervise and delegate tasks to CNAs in accordance with state regulations.
• Compassionate, resident-focused approach to care with a commitment to preserving dignity and quality of life.
• Current CPR/BLS certification.

Responsibilities

• Record a patient’s medical history accurately.

• Monitor patients’ condition including fluid intake and output and compose patient charts.
• Assist ailing patients in daily necessary activities such as eating and bathing.
• Administer injections, prescribed medications, enemas, etc., clean and dress wounds and assist with other basic medical care tasks.
• Collect samples for lab testing as assigned.
• Provide emotional and psychological support when needed.
• Communicate with patients’ family or friends to provide advice, comfort and release instructions.
