Job summary

Scottish Pines Rehabilitation is seeking a compassionate LPN/LVN to join our long-term care team. You will record histories, monitor vital signs, and assist with daily activities while supporting a resident-centered approach to care.

You will administer medications, participate in care planning, and communicate changes to the interdisciplinary team. A valid LPN/LVN license and BLS/CPR certification are required, with benefits and growth opportunities available.

Qualifications

• Graduate of an accredited Practical Nursing program.
• Current, unrestricted LPN/LVN license in state of practice.
• Knowledge of long-term care regulations and resident rights.
• Experience providing direct patient care to geriatric populations.
• Ability to administer medications safely and accurately.
• Experience with interprofessional teams and charting.

Responsibilities

• Record a patient’s medical history accurately.
• Take and record vital signs like blood pressure, temperature, heart rate.
• Monitor patients’ condition and report changes to the care team.
• Assist with activities of daily living and basic care tasks.
• Administer prescribed therapies and wound care as directed.
