Job summary
Total Life seeks nurse practitioners to lead longevity days at partner senior living communities in the Kansas City area. This in-person contract role begins with educational talks for residents, followed by 45–60 minute longevity visits, with support from our CMO.
Travel to assigned communities is required, and you’ll document encounters per Medicare standards using our EHR. You will be trained on geriatric assessment tools (Mini-Cog) and cognitive screening, with hours typically 4–8 per site
Qualifications
- Experience caring for adults 65+
- Experience with Medicare documentation and E/M coding
- Active Missouri APRN license and RN license
- National NP board certification and DEA registration
- Ability to collaborate with physicians and on-site onboarding
- Willingness to travel to Kansas City-area senior communities
Responsibilities
- Conduct 45–60 minute longevity visits with comprehensive geriatric assessment
- Lead educational talks for residents and perform follow-up visits
- Coordinate with primary care providers for follow-up care and referrals
- Document encounters accurately per Medicare standards in the EHR
- Represent Total Life professionally with residents, families and staff
