Job summary

Eventus WholeHealth is seeking licensed clinicians (LCSW/LISW/LPCC/LMHC/LMFT) to provide recovery-oriented rehabilitation services in long-term care settings, including skilled nursing and assisted living. You will assess residents, develop treatment plans, and collaborate with a multidisciplinary team to improve quality of life for residents.

Qualifications

• Knowledge of assessment techniques and psychotherapy methods.
• Knowledge of DSM and regulatory standards governing psychotherapy.
• Ability to document treatment plans and clinical notes.
• Strong written and verbal communication skills.

Responsibilities

• Assess residents and develop individualized treatment plans.
• Consult with primary care physicians, family, and facility staff as needed.
• Document treatment plans and psychotherapy notes within required timelines.
• Maintain open communication with facility staff and leadership.
• Adhere to HIPAA and all regulatory guidelines.
