Job summary

Eventus WholeHealth seeks licensed mental health clinicians to provide recovery-oriented rehab services in long-term care settings, including skilled nursing and assisted living. You will join a multidisciplinary team to support adults with anxiety, depression, and other mental health needs.

The role emphasizes treatment planning, collaboration with facility staff, and adherence to HIPAA and regulatory standards.

Qualifications

• Master’s degree in social work and current license to practice in the state.
• Licensure as LCSW/LISW/LPCC/LMFT or equivalent required.

Responsibilities

• Assess residents and develop individualized treatment plans.
• Consult with primary care physicians, family, facility staff, and support staff as needed.
• Document treatment plans and psychotherapy sessions within 48 hours.
• Review care needs with facility staff and communicate findings.
• Maintain HIPAA and regulatory compliance in all activities.
• Consult with supervisor when facing unfamiliar requests.
