Job summary

The Community Agency for Senior Citizens (CASC) seeks an Outreach Information Specialist to provide objective, unbiased long-term care information to clients, professionals, and caregivers in Staten Island. The role supports the NY Connects program and assists with outreach and resource updates.

Responsibilities include screening for program eligibility, assisting with public benefits enrollment, and representing CASC at community events, with some evening and weekend hours required.

Qualifications

• BSW/BS/BA level education preferred.
• Experience in health/human services with older adults, disabled, behavioral health, or substance use populations.
• Familiarity with Medicare, Social Security and NYC HRA.
• Knowledge of Microsoft Word, Excel and Outlook.
• Bi-lingual (Spanish-English) preferred.
• Must have a car for travel.
• Experience conducting outreach at community events.

Responsibilities

• Provide objective and unbiased information on long-term care options for clients, professionals and caregivers.
• Conduct No Wrong Door screening by phone or in person for eligibility.
• Assist with enrollment for Medicaid and public benefits as appropriate.
• Provide Person Centered Counseling when appropriate.
• Assist with Care Transitions as needed.
• Public speaking for outreach activities and professional/consumer groups.
• Assist with Outreach activities and resource updates with the Program Director.
• Maintain CASC/VIVE documentation and complete required statistical records.
• Participate in required trainings and team meetings.
• Some evenings and weekends may be required.
