Job summary

The Community Agency for Senior Citizens (CASC) seeks an Outreach Information Specialist to provide objective, unbiased long-term care information to clients, professionals, and caregivers in Staten Island. The role supports the NY Connects program and assists with outreach and resource updates.

Responsibilities include screening for program eligibility, assisting with public benefits enrollment, and representing CASC at community events, with some evening and weekend hours required.

Qualifications

  • BSW/BS/BA level education preferred.
  • Experience in health/human services with older adults, disabled, behavioral health, or substance use populations.
  • Familiarity with Medicare, Social Security and NYC HRA.
  • Knowledge of Microsoft Word, Excel and Outlook.
  • Bi-lingual (Spanish-English) preferred.
  • Must have a car for travel.
  • Experience conducting outreach at community events.

  • Public speaking for outreach activities and professional/consumer groups.
  • Assist with Outreach activities and resource updates with the Program Director.
  • Maintain CASC/VIVE documentation and complete required statistical records.
  • Participate in required trainings and team meetings.
  • Some evenings and weekends may be required.

Purpose and Objectives:

Provide objective and unbiased information and assistance on all long-term care options, services/supports and provide follow-up to clients, professionals and caregivers. Assist Program Director with Outreach activities and keeping up to date with resources.

We do this by working hand-in-hand with older adults, respecting their right to make decisions for themselves, honoring their life experiences and heritage, and recognizing the importance of friends, relatives and community in creating supportive environments for people as they age.

NY Connectsis a state–wide program providing objective and unbiased information and assistance to individuals with disabilities of ALL ages with long term care needs or questions, to obtain free and objective information about long term care services and supports over the phone, via email, and in person (onsite and offsite). Information/assistance can be sought by individuals themselves or their caregivers, friends, neighbors or helping professionals.

Position Qualifications:

BSW, BS, BA level education is preferred.

Must have work experience in health/human/social services working with older adults, developmentally disabled, physically disabled, behavioral health and/or substance abuse populations.

Familiarity with Medicare, Social Security and Human Resources Administration NYC.

Knowledge in Microsoft Word, Excel and Outlook is required.

Bi-lingual (Spanish-English) preferred, but other languages are welcomed as well.

Must have a car for travel for work.

Experience conducting outreach/tabling at community events

Responsibilities:

Be responsible for providing objective and unbiased information and assistance on all long-term care options, services/supports and provide follow-up to clients, professionals and caregivers.

Conduct No Wrong Door screening with individuals over the phone, in-person, in order to obtain information for both possible programmatic and financial eligibility for various programs.

Application and enrollment assistance for public benefits, including Medicaid as appropriate.

Will provide Person Centered Counseling when appropriate.

Will provide assistance with Care Transitions as appropriate.

Public speaking for outreach activities and other professional and consumer groups needing long-term care information as assigned.

Assist Program Director with Outreach activities and keeping up to date with resources.

Participate in special projects as assigned by Program Director.

Will follow all NYC DEPT. FOR THE AGING Program Standards.

Maintain case records on NYC DEPT. FOR THE AGING VIVE Client Database for all clients and complete necessary statistical documentation.

Will participate in mandated State/DEPT. FOR THE AGING funded trainings and any additional trainings as assigned.

Other duties as assigned.

Some evening and weekend hours will be required.

Requisite Skills and Capabilities:

Knowledge of Microsoft Office, including Word, Excel, Outlook required.

Organized: ability to manage time, job responsibilities and prioritize tasks appropriately.

Excellent written and verbal communication skills, detail-oriented.

Supervised by NY Connects Program Director.

Participate in team meetings.

Participate in DEPT. FOR THE AGING and CASC trainings and education sessions.

Participate in local educational and informational sessions and conferences.

Salary and Benefits:

Salary: $47,000.00 - $49,000.00 per year
