Job summary

Hudson Guild in New York City seeks a NY Connects Specialist to provide free, objective information about long term services and supports to individuals, caregivers and families. Under the supervision of the Manhattan NY Connects Program Manager, the Specialist will answer calls, perform data entry, complete reports and help with outreach and counseling.

Qualifications include a Bachelor's degree in Social Work or related field, 1–2 years in health and human services, and proficiency with Google

Qualifications

• Bachelor’s degree in Social Work or related field.
• 1–2 years’ experience in health and human services including LTSS.
• Proficiency with Google Suites and Microsoft Office.
• Experience answering calls in a call center environment; strong customer service and communication skills.

Responsibilities

• Answer calls and provide caller assistance with long term services and supports.
• Data entry into PeerPlace for all services provided.
• Complete weekly/monthly reports on assigned deliverables.
• Screen for benefits and entitlements; assist with applications and advocacy.
• Assist clients in accessing services, including referrals.
• Provide Person-Centered Options Counseling.
• Conduct in-home visits to assess status and assist with entitlements.
• Maintain complete and accurate case records within time frames.
• Coordinate translation services as needed.
• Participate in supervision and staff meetings.
• Support program planning and development with the Program Director.
• Contribute resources to the Resources Directory and NY Connects Newsletter.
• Perform other duties as assigned.
