Job summary

CINQCARE seeks an Outreach/Care Guide to bridge the human and medical sides of care in local communities. You’ll listen, connect, and elevate stories to help Family Members feel supported on their journey.

Role combines outreach presence with wraparound support, collaborating with nurses, social workers, and the care team to ensure access to services and build trust with families in need.

Qualifications

• Associate degree or equivalent; bachelor’s degree preferred.
• 1–3 years in outreach, health education, or related work.
• Valid driver’s license and reliable transportation.
• Background check and training prior to field deployment.
• Bilingual or multilingual a plus (e.g., Spanish, Haitian Creole, Russian).
• EMT or Social Worker experience preferred.

Responsibilities

• Engage Family Members where they live, worship, and gather.
• Build trust through consistent presence and culturally aligned communication.
• Surface and address barriers to care such as food insecurity, transportation, or housing.
• Assist with paperwork for social services and basic support needs.
• Support in-home technology setup to enable virtual visits.
