Access Medical Clinic seeks a Long-Term Care Advanced Practitioner (APP) to serve as a dedicated clinical partner in skilled nursing, LTC, and post-acute facilities. The APP provides advanced clinical management and supports resident care alongside the medical director and interdisciplinary teams.

Qualifications

• MSN, DNP, MPAS, DMSc or equivalent advanced degree.
• Active license in the relevant state.
• Board Certification (AANP/ANCC/NCCPA) with DEA and BLS certification.

Responsibilities

• Perform comprehensive and ongoing clinical assessments for long-term care residents.
• Diagnose and manage acute and chronic LTC diseases.
• Order and interpret diagnostic, laboratory, and radiographic data.
• Develop, implement, and adjust individualized care plans with facility teams.
• Provide advanced management of high-risk LTC conditions (infection, wounds, CHF/COPD, dementia).
• Support readmission reduction and clinical quality improvement initiatives.
• Educate and support facility nursing staff on care protocols.
