Job summary

Longevity Health is seeking an Advanced Practice Provider (NP or PA) to manage a panel of nursing home residents under the Institutional SNP Model of Care. The APP will deliver collaborative primary care, develop individualized care plans, and participate in facility care teams to align with member goals.

Qualifications

• 6+ months experience as a Nurse Practitioner or Physician Assistant.
• Experience in nursing home environment strongly desired.
• Computer/technology proficiency required.
• Ability to engage and communicate effectively with physicians, members, and family members.
• Self-directed but functions well in a team environment.
• Excellent clinical skills—assessment, diagnosis, treatment, and prescribing for chronically ill population.

Responsibilities

• Provides collaborative primary care and services to a panel of Longevity Health Plan members.
• Executes and delivers Model of Care activities and care planning.
• Facilitates prior authorization, A/B services, and medication reviews; closes quality gaps (HEDIS/STARs).
• Communicates with PCPs, residents, families, and care teams regarding care plans and changes.
• Maintains autonomy and accountability in the nursing home setting and supports market goals.
