Job summary

Access Medical Clinic is seeking a Long-Term Care Advanced Practitioner (APRN or PA) to serve as a dedicated clinical partner in skilled nursing and post-acute facilities. You will provide high-autonomy care, support resident treatment, and partner with facility leadership and medical directors for quality outcomes.

Requirements include an advanced degree (MSN/DNP/MPAS/DMSc or equivalent), active state license, board certification, DEA and BLS, and strong communication skills.

Qualifications

• MSN, DNP, MPAS, DMSc or equivalent advanced degree.
• Active license in the applicable state and maintenance in good standing.
• Board Certification (AANP/ANCC/NCCPA).
• DEA and BLS certification.
• Excellent interpersonal communication skills with patients and staff.
• Knowledge of professional medical practice and patient care.
• Knowledge of organizational policies, regulations, and procedures.
• Knowledge of medical equipment and instruments used in care.
• Ability to interpret, adapt and apply treatment protocol guidelines.

Responsibilities

• Perform comprehensive and ongoing clinical assessments for residents in LTC/post-acute settings.
• Diagnose and manage acute and chronic diseases common to LTC populations.
• Order and interpret diagnostic, laboratory, and radiographic data.
• Develop, implement, and adjust individualized care plans in collaboration with facility teams.
• Provide advanced management of high-risk LTC conditions (infection management, CHF, COPD, dementia).
• Coordinate with interdisciplinary teams and educate facility staff on care standards.
• Assist with admission stabilization and transition-of-care management.
• Support regulatory readiness and quality improvement initiatives.
