Job summary

Access Medical Clinic seeks a Long-Term Care Advanced Practitioner (APN/PA) to provide clinical leadership within skilled nursing and post-acute facilities. You will conduct comprehensive assessments, manage complex LTC conditions, and collaborate with physicians and care teams to optimize resident outcomes.

Ideal candidates hold an MSN/DNP/MPAS/DMSc with board certification and DEA/BLS credentials. Willingness to travel regionally and maintain strict HIPAA standards is required.

Qualifications

• MSN, DNP, MPAS, DMSc, or equivalent advanced degree.
• Currently registered/licensed in applicable State with active license.
• Board Certification (AANP/ANCC/NCCPA) and DEA/BLS certifications.

Responsibilities

• Perform comprehensive and ongoing clinical assessments for LTC/post-acute residents.
• Diagnose and manage acute and chronic LTC conditions.
• Order and interpret required diagnostic data and develop care plans.
• Provide advanced management of high-risk LTC conditions including infection and sepsis risk mitigation.
