Job summary

Longevity Health is seeking a Nurse Practitioner or Physician Assistant to manage a panel of enrolled long-term care members. You will perform comprehensive assessments, develop individualized care plans, and collaborate with a clinical leadership team to align with members' goals and needs.

You'll participate in care team meetings, communicate with families, and support initiatives to optimize quality of life for residents while delivering high-quality, patient-centered care in a nursing home

Qualifications

• 6+ months experience as a NP or PA.
• MS in Nursing as a NP from a nationally accredited program OR Master’s Degree from a nationally accredited program.
• Experience working in the nursing home environment is strongly desired
• Strong communication with physicians, members and families
• Self-directed but functions well in a team environment
• Excellent clinical skills—assessment, diagnosis, treatment, prescribing for chronically ill
• Demonstrated critical thinking, organization, and prioritization

Responsibilities

• Provides collaborative primary care to a panel of members
• Delivers ISNP Model of Care activities and advanced care planning
• Manages prior authorization, Part A and B services, medication reviews and quality gap closure
• Serves as trusted advisor to members, loved ones and nursing home staff
• Communicates with members’ PCPs, loved ones and interdisciplinary care team
• Supports overall market goals and balances member caseloads
• Promotes Longevity Health with members and facilities
• Functions autonomously and is accountable for practice
• Establishes meaningful relationships in the nursing home environment
• Performs other related duties as assigned
