Qualifications

• Active, unrestricted license as an Advanced Practice Nurse or Physician Assistant.
• Minimum three years in clinical practice with leadership experience preferred.
• Experience in nursing facilities or long-term care settings is strongly preferred.

Responsibilities

• Lead and oversee clinical teams to ensure model of care adherence.
• Coordinate with facilities, PCPs, and APPs to optimize patient care for ISNP residents.
• Oversee chart reviews, MTM, HEDIS/STARs reporting, and regulatory compliance.
