Job summary

Access Medical Clinic seeks a Long-Term Care Advanced Practitioner (APRN or PA) for part-time, in-person clinical support across skilled nursing and post-acute facilities in Warren, Arkansas. You will stabilize complex residents, reduce hospital transfers, and partner with facility teams to improve outcomes and regulatory readiness.

The role requires licensure in the state, board certification, DEA and BLS, and strong communication with residents and families.

Qualifications

• MSN, DNP, MPAS, DMSc, or equivalent advanced degree required.
• Board Certification (AANP/ANCC/NCCPA) required.
• DEA and BLS certifications required.

Responsibilities

• Perform comprehensive and ongoing clinical assessments for long-term care residents.
• Diagnose and manage acute and chronic LTC conditions.
• Order and interpret diagnostic data (labs, imaging).
• Develop and adjust individualized care plans with facility teams.
• Manage high-risk LTC conditions (infection, wounds, CHF, COPD).
• Engage in end-of-life discussions when appropriate.
• Collaborate with facility leadership to optimize workflows and transitions.
• Support readmission reduction and quality improvement initiatives.
