Job summary

My Access Medical Clinic is seeking a Long-Term Care Advanced Practitioner (APP: APRN or PA) to provide continuous clinical presence in skilled nursing and post-acute facilities. The role emphasizes autonomous practice and collaboration with physicians, nursing staff, and leadership to enhance resident care and reduce avoidable hospital transfers.

The APP will lead assessments, care planning, and complex case management, including infection control, wound care, and palliative discussions.

Qualifications

• MSN, DNP, MPAS, DMSc or equivalent advanced degree.
• Currently registered/licensed in applicable State with an active license.
• Board Certification (AANP/ANCC/NCCPA).
• DEA and BLS certification.
• Excellent interpersonal communication skills.
• Knowledge of medical practice and patient care.
• Ability to interpret and apply treatment protocol guidelines.

Responsibilities

• Perform comprehensive and ongoing clinical assessments for long-term care and post-acute residents.
• Diagnose and manage acute and chronic disease processes common to LTC populations.
• Order and interpret diagnostic, laboratory and radiographic data.
• Develop, implement, and adjust individualized care plans in collaboration with facility teams.
• Provide advanced management of high-risk LTC clinical conditions including infection management, sepsis risk mitigation, and wound care.
• Manage CHF, COPD, dementia, and polypharmacy to reduce medication risk.
• Engage in end-of-life and goals-of-care discussions when appropriate.
• Collaborate with facility leadership to optimize workflow and communication.
• Assist with new admission stabilization and transition-of-care management.
• Participate in interdisciplinary care planning and complex case reviews.
