Longevity Health Plan is seeking an experienced Advanced Practice Provider leader (NP/PA) to oversee clinical operations within nursing facilities. You will drive the Model of Care, coordinate care transitions, and ensure regulatory and quality standards for ISNP residents.

The role emphasizes coaching APPs, conducting chart reviews, and collaborating with PCPs to optimize member outcomes in a fast-paced, regulated environment.

Qualifications

• Active NP or PA with required licenses (state NP license or PA licensure) and applicable board certification.
• Experience in nursing facilities, clinical quality programs, and utilization management is highly desirable.
• Strong leadership and ability to manage multiple projects and deadlines in fast-paced environments.

Responsabilités

• Conduct rounds to contracted nursing facilities to ensure Model of Care delivery and regulatory compliance.
• Lead review meetings with facilities to discuss clinical quality results and improvement opportunities.
• Review daily clinical performance metrics and take corrective action when off-target.
• Provide coaching and coverage for clinical team members and APPs.
