Longevity Health Plan is seeking an Advanced Practice Provider (NP/PA) to deliver primary care for nursing home residents enrolled in the Longevity Health Plan. The APP will develop individualized plans of care, attend care team meetings, and communicate with residents and families to align with goals of care.

The APP conducts comprehensive assessments, documents health status, and follows clinical guidelines, supported by a clinical leadership team to ensure high-quality care for Longevity

Qualifications

• 6+ months experience as Nurse Practitioner or Physician Assistant.
• Master of Science in Nursing as a Nurse Practitioner or Master’s Degree in Nursing.
• Experience in nursing home environment is strongly desired.

Responsabilités

• Provide collaborative primary care to a panel of Longevity Health Plan members.
• Execute Longevity Health Plan Model of Care activities and care planning.
• Communicate with members, families, and care teams about goals of care and changes in condition.
• Support clinical operations functions and quality initiatives such as prior authorization and care coordination.
