Access Medical Clinic is seeking a Long-Term Care Advanced Practitioner (APP) to serve as a dedicated clinical partner in skilled nursing, long-term care, and post-acute facilities. The APP will provide autonomous medical management, collaborate with facility leadership, and improve resident safety, communication, and care quality.

Qualifications

• Excellent interpersonal communication skills required.
• Ability to exercise strong independent clinical judgment within licensure.
• Knowledge of professional medical practice and patient care protocols.
• Familiarity with EHR systems and clinical documentation.

Responsibilities

• Perform comprehensive clinical assessments for long-term care residents.
• Diagnose and manage acute and chronic LTC conditions.
• Order and interpret diagnostic, laboratory, and radiographic data.
• Develop and implement individualized care plans with facility teams.
• Provide advanced management of high-risk LTC conditions (infection, wound care, CHF/COPD).
• Support transition of care and interdisciplinary care planning.
