Job summary

Qualifications

• State licensed Advanced Practice Nurse or Physician Assistant with NP/PA leadership experience.
• DEA license and active NPI/Medicare provider numbers are required.
• Minimum of three years in APN/PA roles with leadership and clinical quality program experience.
• Strong communication, teaching, and public speaking skills for team development.

Responsibilities

• Lead and mentor APP team, provide onboarding and performance feedback.
• Oversee Model of Care implementation, coordinate with facilities and PCPs.
• Develop action plans to improve HEDIS/STARS metrics and MTMs.
• Conduct chart audits and ensure compliance with regulatory requirements.
• Manage education, training, and supervision of nursing facility staff.
