Job summary

Access Medical Clinic seeks a Long-Term Care Advanced Practitioner (APRN or PA) to serve as a dedicated clinical partner in skilled nursing, long-term care, and post-acute facilities. The APP will work with facility leadership and interdisciplinary teams to deliver high-quality resident care.

Responsibilities include comprehensive assessments, complex disease management, and transitioning care with emphasis on reducing hospital transfers and improving clinical outcomes.

Qualifications

• MSN, DNP, MPAS, DMSc, or equivalent advanced degree.
• Active state license and board certification (AANP/ANCC/NCCPA).
• DEA and BLS certification.
• Excellent interpersonal communication skills required.
• Ability to work independently within licensure scope.

Responsibilities

• Perform comprehensive clinical assessments for long-term care and post-acute residents.
• Diagnose and manage acute and chronic disease processes common to LTC populations.
• Order and interpret necessary diagnostic, laboratory and radiographic data.
• Develop, implement, and adjust individualized care plans with facility teams.
• Provide advanced management of high-risk LTC clinical conditions including infections, wounds, CHF, COPD, dementia, polypharmacy and end-of-life discussions.
• Support facility regulatory readiness and quality improvement initiatives.
