Job summary

Qualifications

• LPN/LVN licensed in the practicing state.
• BLS/CPR certification or willingness to obtain one.
• Experience in long-term care settings is required or preferred.

Responsibilities

• Follows established procedure for charting and incident reporting.
• Follows company procedures for narcotic counting, pharmacy orders, and restraint reduction.
• Documents any change in patient's conditions including nutritional issues, infections, skin conditions, weight loss, or other noticeable changes.
• Uses the Quality Life Manual to identify, place, and monitor patients in appropriate programs.
• Documents and notifies applicable departments of room changes, LOA, new admits, or discharges.
• Makes rounds with physicians when necessary.
