Job summary

Sweetwater Care is seeking an LVN/LPN to provide nursing care within scope of practice in a long-term care setting. The role requires a valid LVN/LPN license in California and current BLS/CPR certification, with preference for nursing management experience.

The candidate will follow established charting procedures, assist with narcotic counting and pharmacy orders, and document changes in patient conditions while collaborating with physicians and other departments to ensure high-quality care.

Qualifications

• Long Term Care experience.
• Point click care charting experience.
• Current LVN/LPN license in practicing state.
• Current BLS/CPR certification or willingness to acquire one.
• Experience in nursing management is preferred but not required.

Responsibilities

• Follows established procedure for charting and incident reporting.
• Follows company procedures for narcotic counting, pharmacy orders, and restraint reduction.
• Documents any change in patient's conditions including nutritional problems, infections, skin conditions, weight loss, or other noticeable changes.
• Utilizes the Quality Life Manual for identification, placement, and monitoring of patients in the appropriate programs.
• Document and notify applicable departments of any patient's room changes, LOA, new admits, or discharges.
• Makes rounds with physicians when necessary.
