Job summary

Sweetwater Care in California is seeking a licensed Vocational Nurse/Licensed Practical Nurse to provide nursing care within the scope of practice and ensure patient needs are met per standards and physician orders.

The role involves charting, incident reporting, and following policies on narcotics, medications, and patient safety. Experience in long-term care and familiarity with PointClickCare are preferred.

Qualifications

• Experience in long-term care settings.
• Current LVN/LPN license in practicing state (California).
• BLS/CPR certification or willingness to obtain.

Responsibilities

• Follows established procedure for charting and incident reporting.
• Follows company procedures for narcotic counting, pharmacy orders, and restraint reduction.
• Documents any change in patient\'s conditions including nutritional problems, infections, skin conditions, weight loss or other changes.
• Utilizes the Quality Life Manual for identification, placement, and monitoring of patients in the appropriate programs.
• Document and notify applicable departments of any patient\'s room changes, LOA, new admits, or discharges.
• Makes rounds with physicians when necessary.

POSITION DESCRIPTION

QUALIFICATIONS

• Point click care charting experience
• Current LVN/LPN license in practicing state.
