Job summary

Sweetwater Care in Visalia, CA is seeking a licensed LVN/LPN to provide nursing care within the facility's guidelines and physician orders. The role requires long-term care experience, familiarity with PointClickCare charting, and a current CA license, with BLS/CPR certification either in place or willingness to obtain.

Responsibilities include charting per policy, narcotic counts, and timely documentation of patient condition changes, along with physician rounds when necessary.

Qualifications

• Long-term care experience required in a nursing facility.
• Current LVN/LPN license in California.
• BLS/CPR certification required or willingness to obtain.
• Experience in nursing management is preferred but not required.

Responsibilities

• Follows established procedures for charting and incident reporting.
• Manages narcotic counting, pharmacy orders, and restraint reduction per procedures.
• Documents changes in patient conditions including nutrition, infections, skin issues, weight changes, or other notable shifts.
• Uses Quality Life Manual to identify, place, and monitor patients in appropriate programs.
• Notifies relevant departments of room changes, LOAs, admissions, and discharges.
• Rounds with physicians when needed.

POSITION DESCRIPTION

Provides nursing care within scope of practice to ensure patient’s needs are met in accordance with standards of practice, physician orders, center policies as well as state, federal and local guidelines.

QUALIFICATIONS

• Long Term Care experience
• Point click care charting experience
• Current LVN/LPN license in practicing state.
• Current BLS/CPR certification or willingness to acquire one.
• Experience in nursing management is preferred but not required.

RESPONSIBILITIES

• Follows established procedure for charting and incident reporting.
• Follows company procedures for narcotic counting, pharmacy orders, and restraint reduction.
• Documents any change in patient's conditions including but not limited to nutritional problems, infections, skin conditions, weight loss or any other noticeable changes.
• Utilizes the Quality Life Manual for identification, placement, and monitoring of patients in the appropriate programs.
• Document and notify applicable departments of any patient's room changes, LOA, new admits, or discharges.
• Makes rounds with physicians when necessary.
